Anyone who lives in zip codes 3082 or 3083 or 3084 is welcome to register as a patient at one of the Carnisse House family medicine practices. You will be enrolled at the practice that currently has the most space for new patients. If you have a preference for a particular doctor, please indicate this in "comments".
If you have completed this form, your registration is not yet complete. You cannot make an appointment or request medication until you have completed the following steps:
- Come to the counter with your ID card or passport. This is possible from 3 working days after filling in this form. The desk is open Monday to Friday from 8:00 to 11:00 and from 14:00 to 16:30.
- Please give your previous general practitioner permission to forward your file to us. After we have received your file, we can schedule an appointment for your consultation.
