How do I contact the practice?

With this message, we would like to let you know in what ways you can contact your general practice in the Carnissehuis.

In order to be able to continue to help you in the best possible way, we have changed the way we work. This was necessary because our practice is getting busier.

First you can read how to reach us. Below that is more information on how to use the digital options mijngezondheid.net (MgN) and the MedGemak app.

Emergency cases

  • For emergencies, the doctor's practice is available Monday through Friday from 8:00 - 17:00. In case of a life threatening situation press 1.

Ordering repeat medication

  • Digital: You can easily repeat your medication via MgN and the MedGemak app.
  • Repeat service from the pharmacy: The pharmacy automatically requests your medicines from us, you can register for this at the pharmacy.
  • By phone: Call the number of your general practice and press 2. You can say the medicine you need.
  • Mailbox: Put the old box or a note with the medication you need (note: your name and date of birth must be on it!) in the mailbox at the practice.

Ask a question

  • Digital: You can send your GP a message with attachment (e.g. photo) and ask your question via MgN and the MedGemak app. You can also view your file and test results here.
  • By telephone to the assistant: Call the number of the general practice and ask your question to the assistant.
  • By telephone to the GP: You can schedule a call-back appointment with your GP via MgN and the MedGemak app. The GP will then call you back at the agreed time.

Make an appointment

  • Digital: You can use MgN and the MedGemak app to schedule an appointment with the assistant, for example:
    • blood pressure measurement
    • a contraceptive pill
    • Spraying out ears
    • wound care
    • nitrogen warts
    • take a smear test for the population screening
  • By telephone: If the assistant or doctor considers it important (after digital or telephone consultation) for you to come to the consultation, we will let you know. You can then schedule an appointment with the GP by telephone.

Registration in practice

  • Step 1: Fill in the registration form. You can print this from www.carnissehuis.nl or collect it from the reception desk.
  • Step 2: Take the registration form with you and identify yourself at the counter. This is possible Monday to Friday from 8:00-11:00 and from 13:30-16:30.

Visiting the practice

The reception desk is open Monday to Friday from 8:00-11:00 and from 13:30-16:30. You are only welcome at the desk for:

  • Registering for an appointment, this can be done from 5 minutes before the appointment. Do not come earlier, we have limited space in the waiting room.
  • Handing in urine.
  • Collecting referrals or material for research if this has been agreed with you in advance.
  • Registration in the practice.

For all other questions, to repeat medication and to make an appointment, please refer to the previously mentioned possibilities.

Mijngezondheid.net and the MedGemak app

You can log on to mijngezondheid.net on our website www.carnissehuis.nl/huisartsen and download the MedGemak app.

Via this MgN website and MedGemak app you can:

  • View your medication history and repeat your medication.
  • Send your doctor a message with a question and possibly a photo, so that the doctor can already take a look at, for example, a changed piece of skin or complaints about the eye. You can also read messages from the GP.
  • View your file, blood results and other results.
  • Schedule an appointment with the assistant
  • Scheduling a telephone appointment with the family doctor
  • Report changes in your contact details, such as your home address, e-mail address or telephone number.

What do we ask of you?

We would like to ask you to use the website and the app as much as possible. This will avoid long waiting times on the phone and you will get an answer to your question faster. We ask you to log in at least once to the website www.mijngezondheid.net and/or to the app MedGemak. Below you can read how to do that.

If you have any questions or comments, or if you have trouble logging on to www.mijngezondheid.net or installing the MedGemak app, please let us know. The assistant can help you further.

Step 1: Apply for DigiD

If you don't have a DigiD yet, you can apply for one at: https://digid.nl/aanvragen

For your children under 12, you can use your own DigiD.

This is also possible for your children between the ages of 12 and 16, but you will need your child's permission to see his/her file. Make good agreements about this together.

Step 2: Logging into mijngezondheid.net (MgN) with your DigiD

Step 3: Download MedGemak app, log in with activation code mygezondheid.net (optional)

Step 4: Registration National Switch Point (LSP) Via the national switch point, healthcare providers can, if necessary, view the most important data from your file outside office hours (e.g. at the GP Post). You can give permission for this at https://www.volgjezorg.nl. We will then process this in your file.
